Your Benefits Administration Team will assist you with the plan’s day-to-day administration:

  • Adding new employees/terminating existing employees; adding/removing dependents and beneficiaries
  • Assistance using the Ariel Benefits Online website
  • Benefit plan coverage and eligibility inquiries
  • Premium statement inquiries/billing questions
  • Waiving plan benefits or waiting periods

The Early assistance form is to be submitted to Desjardins after 10 days of absence. The Desjardins team will contact the member and provide you with an update.

Use Certificate # that combines the division # and the member's date of birth – DIV/YY/MM/DD
